Online Health History Forms (On-Demand)

With Sesame Online Health History Forms, your office will be able to receive patient health and insurance information prior to their first appointment. The forms are submitted directly into your existing Sesame Member Dashboard for you to save to your computer, attach to a patient file, or print in office. This is a HIPAA compliant way to send and receive electronic patient forms.

Our training covers how to customize the forms for your office (starting from a standard template), posting the form to your website and emails, and retrieving completed forms. To see an example form, click here
